Required Doctoral Forms Timeline

Talk to your graduate program coordinator (GPC) and follow the process below to submit forms required for all doctoral students.

Review the full list of doctoral forms for other forms you may need to submit.

Step 1

Submit the Supervisory Committee Approval form by the end of your third semester.

Step 2

Submit the Program of Study form by the end of your second or third semester, after the Supervisory Committee Approval form.

Step 3

Submit the Application for Candidacy as soon as you finish your comprehensive exams and at least three months prior to your final defense.

Step 4

Submit the Appointment for Exam form at least 10 business days before your final defense. All previous forms (Supervisory Committee form, Program of Study, and Application for Candidacy form) must be approved before the Appointment for Exam can be processed.

Step 5

Your GPC will submit the Record of Exam form after your exam. You must not handle the Record of Exam form once it is completed.

Step 6

Submit the Format and Style form and Authorship and Copyright after your defense, before you are ready to begin the review process.

Step 7

Once you complete all dissertation requirements, consult with your GPC for department-specific review procedures and have your GPC submit your dissertation to the School of Graduate Studies for review.
