Professional/Plan C students should talk to their department about the need for this form.

Due Date

Master’s students: By the end of your second semester.
Doctoral students: By the end of your third semester.

Description

A Supervisory Committee Approval form confirms the individuals who will serve on your committee.

Your Supervisory Committee Approval Form should approved before the semester in which you defend. No initial or revised Supervisory Committee forms should be submitted within the six weeks prior to your final defense.

You, your GPC, major professor, committee members, department head, and the vice provost of graduate studies will receive a notification from ServiceNow to review and approve the Supervisory Committee Approval form. Once all the required approvals have been completed, you will receive a notification indicating that your committee has been approved.

Master’s Plan A/B committee requirements

Required committee members (must be indicated on the form):

  • Major Professor (or advisor) – must be from within the department issuing the student’s degree
  • Committee member – may be co-chair if necessary and approved
  • Outside member – outside area of emphasis, may be from within the student’s department

A master’s Plan A or B committee must include three faculty members who have been approved to serve as graduate faculty by your department head and college dean. The major professor (or advisor), and one committee member must be from your department, and the outside member must be from outside your area of emphasis (but may be from your department). You may have additional committee members if necessary. If the form does not have all required committee roles filled, it will be denied.

Doctoral Committee Requirements

Required committee members (must be indicated on the form):

  • Major Professor (or advisor) – must be from within the department issuing the student’s degree
  • 3 Committee members – 1 may be co-chair if necessary and approved
  • Outside member – must be from outside the student’s department

A doctoral degree supervisory committee must include five faculty with doctoral degrees who are approved by your department head and college dean. Three members must be from within and at least one must be from outside your department or interdepartmental degree-granting program. You may have additional committee members if necessary. If the form does not have all required committee roles filled, it will be denied.

Professional/Plan C Requirements

If you’re a Professional/Plan C student, ask your department if a Supervisory Committee Approval form is required.

Concurrent Student requirements

If you are a concurrent student, please work with your department(s) to ensure all committee requirements specific to your concurrent programs are met.
