This form is in accordance with interim USU Policy 340. We are required to report incidents involving an alleged violation of interim USU Sexual Misconduct, Policies 339, or 339A, which include: sexual harassment, relationship violence, sexual assault, sexual exploitation, and sex-based stalking.
