IRB-Approved Protocol Closeout

Description

If you have approval from the Institutional Review Board (IRB) and your IRB-approved protocol is ready to close, please submit a status report.

• If your research protocol received an exemption, please complete the Exempt Status Report.
• If your research protocol received expedited or full board approval (approved under a General application), please complete the General Status Report.

If you are unsure which form to fill out, contact the IRB office with your protocol number at (435) 797-1821.
