Academic Forms
USU Graduate School forms were created to track your degree progress, facilitate approvals, aid exceptions, and more. Please always consult with your graduate program coordinator on which forms are required for your degree and situation.
When filling out forms, read the description and instructions first. If the form is not correct when submitted, it will have to be canceled and resubmitted.
Find Your Form
Application for Candidacy
Submit an Application for Candidacy at Utah State University after your dissertation proposal is approved, all regulatory approvals are complete and you pass your comprehensive exam.
Appointment for Examination
Submitting the Appointment for Examination form allows the School of Graduate Studies to confirm that all required paperwork is complete and your committee members have read the thesis/dissertation ahead of time.
Authorship and Copyright
The Authorship and Copyright form specifies plans for publication of your thesis/dissertation.
Change Department or Degree Transfer Request
Form used to change your graduate department, degree, or plan type.
Concurrent Degrees (CD)
Use the Concurrent Degree form to begin taking credits toward a second degree while completing your current degree program.
Degree Advancement
Use the Degree Advancement form to enroll in a new graduate degree program without submitting a new admissions application.
Early Commencement
Summer graduates (all plan types) who would like to participate in commencement services the spring semester prior to completing their degree.
Embargo Request
The Embargo form is used to restrict electronic release of your thesis or dissertation.
Full Time at 3 Credits
If you have completed all coursework outlined in an approved Program of Study form, you only need to register for three credits to be considered a full time student.
Graduate Student Leave of Absence
Any graduate (master’s or doctoral) student who plans to take a leave of absence during their program of study.
Graduation Check Request
The graduation check request form indicates that you are ready to graduate.
IRB-approved. Protocol Closeout
If you have approval from the Institutional Review Board (IRB) and your IRB-approved protocol is ready to close, please submit a status report.
Master’s Thesis/Project Approval
The Master’s Thesis/Project Approval form confirms you successfully defended a research proposal and have the necessary regulatory approvals to conduct the proposed research.
Plan B Report/Creative Project Submission
The Plan B Creative Project/Report form grants Utah State University a non-exclusive license to archive your work and make it accessible to the public.
Program of Study
The Program of Study is a contract between you, your committee and the School of Graduate Studies outlining which courses you will take to meet the School of Graduate Studies requirements and complete your degree program.
Readmission
Use the Readmission form if you have not enrolled in courses for over two years.
Reduced Course Load
If you’re an international student, submit the Reduced Course Load form on the ISSS portal to get approval to take less than a full course load.
Split Form
Any undergraduate or concurrent student who wants to pay undergraduate tuition for a course and for that course to be applied to a graduate degree.
Supervisory Committee Approval
A Supervisory Committee Approval form confirms the individuals who will serve on your committee.
Supplemental Language Tuition Form
Any student who wants to seek tuition assistance to support their graduate degree with foreign language coursework which is included on an approved Program of Study.
Thesis/Dissertation Format and Style and Electronic Publication Approval
A Supervisory Committee Approval form confirms the individuals who will serve on your committee.
Title Pages for Master's Plan A and Doctoral Dissertations
The final approval of a student's Master's Plan A Thesis or a Doctoral Dissertation.
Western Regional Graduate Program
Any student who meets the qualifications for the WRGP program and would like to participate in it fill out this form.
Work 20+ Hours
Use the Work 20+ Hours form if you have a .5 FTE and want to work more than 20 hours at Utah State University.
