Change Department or Degree Transfer Request (TR)

Description

Change of Graduate Department:

Students must work with their current Graduate Program Coordinator and the Graduate Program Coordinator of the department they would like to change to if they are changing departments. Once an approval has been made by both departments, the student will initiate the process by submitting the Transfer Request form for official approval by the School of Graduate Studies.

Change of Degree:

Students must work with their Graduate Program Coordinator if they are changing their degree type or program within their current department. Once an approval has been made by their department, the student will initiate the process by submitting the Transfer Request form for official approval by the School of Graduate Studies.

Change of Plan Type:

Students must work with their Graduate Program Coordinator if they are changing their plan type. Once an approval has been made by their department, the student will initiate the process by submitting the Transfer Request form for official approval by the School of Graduate Studies.

Due Date

This form must be submitted before submitting a new Supervisory Committee Approval Form.

Any student who wants to change their graduate department, degree, or plan type.
